Includes information on insured parties, policy coverage, type of property and specific coverage.

Sport Accident Claim Form
Includes instructions on how to properly fill out the form.

Once you have filed out your form(s) you will be required to mail them to the STHF office for membership verification.  Once approved we will submit your claim to All Sport Insurance Marketing Ltd. and they will then be in contact with you regarding your claim.
